Executive summary

A surgery issue of The Lancet provides an update from a 2010 themed issue. Four Research Articles cover endarterectomy, adult epilepsy surgery, cardiac procedures, and surgical care at the end of life. In a three-part Series on organ transplantation, Demonico and colleagues discuss the action needed to increase global organ-donation rates; Jardine and colleagues review the steps that can be taken to limit the effect of cardiovascular diseases and new-onset diabetes after transplantation; and Nankivell and Kuypers review the causes of kidney-graft failure. A recent report by The Royal College of Surgeons of England and a working group of the UK Department of Health: 'The Higher Risk General Surgical Patient: Towards Improved Care for a Forgotten Group', is discussed in an Editorial.
